Why do patients miss appointments?
A booked visit can be lost in four small gaps:
- They forgot. The visit was booked ten days ago and nothing has mentioned it since.
- Plans changed. Moving a visit means phoning in office hours, so some patients just stay away.
- The booking was never confirmed. A slot taken from an ad at night, or by a relative, is a name nobody has checked.
- Nobody learned in time. The doctor waits, and a slot another patient could have taken stays empty.
What is an empty slot worth? Example figures, to replace with yours: an average visit of AED 250, and 3 booked visits missed a day over 25 working days. 3 × 250 × 25 = AED 18,750 of unused doctor time in a month. Meta’s charge for a reminder is a small fraction of that: the rate and a worked clinic month are in our WhatsApp API pricing guide.
What does a WhatsApp reminder flow look like?
Five messages, from booking to the day of the visit; not every patient gets all five. The wording is an example, not an approved template: yours is written with your clinic, in Arabic and English, and you approve it before it goes live.
| When | Example message | How it is sent |
|---|---|---|
| At booking | “Booked: Dr. Hana, Saturday 10:30, Al Majaz branch. Reply YES if you agree that Example Clinic sends you reminders about this visit on WhatsApp.” | A reply inside the open chat window; the clinic keeps the opt-in |
| Day before, 18:00 | “Hi Omar, a reminder of your visit tomorrow at 10:30 with Dr. Hana, Al Majaz branch.” Buttons: Confirm, Change time | An approved utility template with two quick-reply buttons |
| Morning of the visit | “Good morning Omar. We expect you at 10:30 today at the Al Majaz branch.” | A utility template |
| After “Change time” | “No problem. Same doctor: Thursday 17:00 or Saturday 11:30?” | A reply inside the window the tap opened; the old slot returns to the calendar for another patient |
| Deposit, if you take one | “A deposit of AED 100 holds your slot: payment link.” | In the chat; the card is typed on the payment page |
Each morning, reception sees who has not confirmed and calls only those patients. A missed visit moves to “No-show” in the CRM, and a message offers to rebook. Here are the day-before reminder and a change, as an example chat. The AI agent, your clinic’s receptionist on WhatsApp, replies within 5 seconds, day or night:
- The day before, 18:00: the CRM sends the approved reminder.
- Reminder: Hi Omar, a reminder of your visit tomorrow at 10:30 with Dr. Hana, Al Majaz branch. Buttons: Confirm, Change time
- Patient: Change time
- AI agent: No problem. Same doctor: Thursday at 17:00 or Saturday at 11:30?
- Patient: Saturday. Also, should I keep taking my tablets before the visit?
- AI agent: Done: Saturday at 11:30. I can’t advise on medication, so I’ve passed your question to our nurse, who will get back to you tomorrow morning.
- CRM: visit moved, tomorrow’s 10:30 slot open for another patient, medication question sent to the nurse with the whole chat.
Which WhatsApp rules decide how reminders are sent?
Meta’s rules shape the flow, and we write the templates around them:
- The 24-hour window. When a patient writes to you, a 24-hour customer service window opens, and each new message restarts it. Inside it you can reply freely. Outside it, you can send only approved templates. A reminder the day before usually falls outside, so it goes as a template.
- Categories. Meta sorts templates into marketing, utility and authentication. A utility template is non-promotional and specific to the person’s own order, account or service. A reminder of a visit the patient booked fits that description in our reading; Meta decides from the content. Add an offer and it becomes marketing, at a higher price. Keep offers out of reminders.
- Buttons. Utility templates can carry quick-reply buttons. A tap messages you with the button’s text, and a message from the patient opens the 24-hour window, so the agent can offer slots and confirm without another template.
- Approval. Meta reviews every template before you can send it, and says a review can take up to 24 hours. Meta does not translate templates: each language is its own template. We write and submit an Arabic one and an English one, so the patient gets the one in their language.
- Opt-in. Meta requires opt-in before you message anyone. The patient must be told they are opting in, and the clinic must be named. Meta lists a website, SMS, an automated phone menu and a signed paper form among the methods. Keep a record, for example on the patient’s CRM card.
- Quality. Patients can block or report a business, and Meta can pause a template after poor feedback. Send only what the patient needs.
If your clinic still answers from the WhatsApp Business app, the app versus the API shows what changes when reminders run through the API, and how to get the API in the UAE lists the documents.
What should stay out of the chat?
A reminder needs the clinic, the day, the time and the doctor. It does not need a diagnosis, a treatment or a test. Four rules:
- No medical advice. The AI agent gives none. Symptoms, results, medication and “is this normal?” go to your staff with the whole chat, and the agent tells the patient when someone will follow up. Meta’s policy bars using WhatsApp for telemedicine, or to send or request health information, where regulations keep that information out of systems that do not meet heightened health-data requirements. So your clinic decides, under its regulator’s rules, how staff answer. Meta also requires clear routes to a person when automation replies: see menu chatbot versus AI agent.
- Collect only what the booking needs. Name, number, preferred time, doctor or service. Meta’s policy says not to ask people to share personal ID card numbers or other sensitive identifiers. An Emirates ID number is one, in our reading, so take it at reception. For a deposit, the card number goes on the payment page, never into the chat.
- Keep the file elsewhere. Clinical notes, results and prescriptions stay in your clinic’s own system. The CRM holds the booking conversation.
- Your regulator’s rules apply. The health regulator that licenses your clinic sets its own rules for health advertising and patient data. We never label a message “compliant”: your clinic signs off every template and answer.
Reminders are not promotion. If you also send offers, read Meta’s policy first: it lists “medical and healthcare products” among the regulated or restricted goods businesses may not promote on WhatsApp.
How do you tell whether it is working?
Compare your own numbers, not a promise. We do not quote a percentage, because clinics differ. For the month before the flow and the month after, count per doctor:
- visits booked and visits confirmed;
- visits attended and visits missed;
- missed visits per branch and per ad that brought the patient.
Example counts, not a benchmark: 200 visits booked, 180 attended, 20 missed. 20 ÷ 200 = 0.10, so 10 visits missed per 100 booked. Compare that figure month to month, per doctor. The CRM’s stages (booked, confirmed, attended, no-show) give the counts. The reminders and stages run in the WhatsApp CRM, the replies come from the AI sales agent for WhatsApp, and a deposit is a WhatsApp payment link.
